Hey all,
Any idea what may be casuing this? I have one line in my form for users to type in their last name, and randomly, not every form does it on every computer, but sometimes the form shows up without a text box for users to enter their name. See the screenshot below for what it looks like, but not sure whats wrong here, again happens randomly and I just upgraded to SP3, this happened on SP2 as well.
